Kentucky Sports Medicine Clinic: Making An Appointment

Regular Appointment Hours
Patients are seen Monday through Friday, 8:00 a.m.–5:00 p.m.
KSM accepts patients from referring physicians and direct patient calls.
Appointments can be made during these hours by calling: (859) 268-0268.

Initial Visit — you must bring all of the following to your first appointment:

• Insurance card
• Any related x-ray films AND written x-ray reports
• Any related MRI films AND written MRI reports
• Any related CT scan films AND written reports
• Any prior related Surgery Notes, if surgery was performed by another physician
  NOTE: Prior Surgery Notes will need to be faxed to 859-268-4519 prior to visit, for physician review

Arrive 30 minutes prior to your scheduled appointment time
Dress appropriately: shorts for knee exams, tanktops for shoulder/arm exams

Doing Your Part
If you become a KSM patient, you should be involved in all aspects of your own health care.

READ all KSM brochures and other printed materials you are given very carefully.
UNDERSTAND your rights, your responsibilities, your anticipated healthcare costs, and your insurance coverage.
ASK QUESTIONS of the doctors and staff.
BE SURE you understand what doctors and staff tell you about your diagnosis, treatment, and rehabilitation.
TELL US what you want, what you need, and what you expect in your healthcare.
